Comprehensive orthodontics is the most common orthodontic treatment for straightening teeth and gives the best cosmetic and functional long-term results. Comprehensive orthodontic treatment was traditionally only possible with fixed braces, but in many cases can be achieved with both fixed braces and sequential clear aligners, originally pioneered by Invisalign. Both fixed braces and aligners gently guide teeth into their desired positions over certain time periods.
Dental conditions commonly treated with the aid of orthodontics include:
- Crowding (crooked teeth)
- Overbite, underbite, crossbite
- Gummy smile
- Narrow smile
- Thin lips
- Periodontal disease (gum disease such as chronic gingivitis and periodontitis)
- Temporo-mandibular joint (TMJ) disorders and pain
- Bruxism (tooth grinding and clenching)
- Worn teeth
- Sleep apnoea
Traditional orthodontic braces are fixed metal or white ceramic brackets bonded to teeth and linked together with metal wires. The gentle force from the wires helps to align teeth. Elastic bands and other appliances may be used to manipulate the teeth and jaws for improved function. After treatment has completed, retainers (both fixed wires behind teeth and removable plastic retainers) are necessary to ensure teeth do not move back to their starting positions.
Sequential clear aligners are a series of removable clear plastic appliances that are clicked onto and cover the teeth. Each aligner moves teeth a certain amount and hence a series of them are required to achieve alignment. Elastic bands between the top and bottom jaws are sometimes used to improve the bite. Retainers are needed at the end of treatment to prevent teeth from moving back.
In certain cases, both fixed braces and clear aligners can be used in the same orthodontic plan to reduce the time needed for braces.
