Cancellation Policy – Avenue Dental
At Avenue Dental, we’re dedicated to providing the highest level of care and service. To ensure we can offer timely care to all patients, we kindly ask that you notify us if you need to cancel or reschedule your appointment.
Cancellation & Rescheduling Notice:
We require at least 48 hours’ notice if you need to cancel or reschedule. This gives us the opportunity to offer the appointment time to another patient in need.
Late Cancellation:
If less than 48 hours’ notice is given, any holding deposit will be absorbed. A new deposit will be required to secure future bookings.
No-Show (Fail to Attend):
If you do not attend your appointment and do not notify us, it will be considered a Fail to Attend (FTA). Your deposit will be forfeited, and a new deposit will be required to rebook.

Deposit Policy – Avenue Dental
At Avenue Dental, we are committed to providing exceptional dental care using the latest technology and a highly qualified team. To help us allocate the right time, staff, and resources for your care, we require a deposit for specific appointment types.
Deposit Requirements:

Appointments over 60 minutes: $100 deposit

Saturday appointments: $50 deposit

Implants, sedation, or higher-end treatments: $500 deposit
(Note: Sedation deposits may not be transferrable due to anaesthetist involvement)
Late Cancellation or Non-Attendance:

If you cancel with less than 48 hours’ notice, your deposit will be forfeited.

A new deposit will be required to rebook any future appointments.

For standard appointments that are cancelled with more than 48 hours’ notice, your deposit will be transferred to your rescheduled visit.
Deposits are applied to the total cost of your treatment. This policy allows us to manage our schedule efficiently and continue delivering high-quality care to all patients.

Eligibility
To qualify for our Same Day Emergency Guarantee, you must contact us before 10:00am on the day you wish to be seen. -
Location Flexibility
The guarantee is for an appointment at any Avenue Dental location with availability, not necessarily your preferred practice. We will always do our best to accommodate you at your closest Avenue Dental practice, including asking our dentists to stay back or open extra time where possible. If we do not have the capacity or availability at that location, we will offer you an appointment at the next closest Avenue Dental practice with availability. -
Nature of Appointment
Appointments are for emergency dental concerns only and may be limited to initial assessment, pain relief, or temporary treatment, with follow-up appointments scheduled if required. -
Appointment Times
Times may vary based on clinician availability. We will do our best to accommodate you as soon as possible. -
Exclusions
The guarantee does not apply on public holidays, during practice closures, or in circumstances beyond our control (e.g., severe weather events, emergencies). - Scope of Guarantee
The guarantee ensures you will be seen by a dentist for assessment and appropriate emergency care that day. Full treatment may require follow-up appointments but this would be discussed with you once we have seen you and determined what treatment you would need. We of course will get you temporary treatment and/or pain relief (if required on an individual assessed basis) at the emergency appointment.
Our goal is always to see you promptly and provide relief, even if that means attending a nearby Avenue Dental practice.
Promotions (Terms and Conditions)
- Each promotion cannot be used in conjunction with any other promotion and are not substitutable or redeemable for cash.
- To redeem promotion, the patient must mention the promotion when booking the appointment.
- Services will only be provided when suitable and appropriate.
- Additional treatment may be recommended at an additional cost. Any additional treatment and cost will be discussed with you before being carried out.

Avenue Dental – Full Examination (1 hour) Health Fund Only or $95
Details
The Avenue Dental Full Examination (“Full Examination”) is an initial comprehensive dental assessment appointment offered to new and existing patients (if they haven’t seen us in over 3 years) at participating Avenue Dental practices. The Full Examination includes a comprehensive oral health assessment, diagnostic imaging (where clinically appropriate), and treatment planning, subject to the Terms and Conditions outlined below.
Appointment Inclusions
The Full Examination includes the following services, where clinically appropriate:

– A comprehensive dental examination (approximately 60 minutes)
– Bitewing X-rays
– An OPG (panoramic) X-ray
– 3d Scan
– Intra-oral + Extra-oral Photographs
– Oral Cancer Screening
– A personalised treatment plan if treatment is required
The Full Examination is an assessment and planning appointment only and does not include a scale and polish (clean) or any treatment unless otherwise agreed.
Fees and Private Health Insurance
The Full Examination is offered with no out-of-pocket cost for eligible private health fund members, subject to individual policy limits and eligibility. This means we process the appointment through your private health fund using the relevant item numbers, and any applicable rebate is applied directly.
No out-of-pocket means there is no payment required from you at the time of the appointment.
It does not mean the appointment is free.
Patients without private health insurance will be charged a $95.00 fee for the Full Examination.
Important Health Fund Information
Private health insurance rebates vary between funds, policies, and individual circumstances. Avenue Dental:

Does not have access to, and cannot predict, the level of rebate a patient’s health fund may provide

Does not control how benefits are applied or processed by private health insurers
In some cases, the health fund rebate applied may exceed the $95 private fee. You will not be put into credit for the difference. In other cases, patients may prefer to pay privately and reserve their health fund benefits for future treatment.
Patients are responsible for confirming their level of cover directly with their health fund prior to their appointment. Avenue Dental cannot be held responsible for dissatisfaction arising from health fund rebate outcomes.
No Scale and Polish Included
The Full Examination does not include a scale and polish or any cleaning procedures.
This is to allow adequate time for:

A thorough assessment of gum and oral health

Identification of any underlying dental concerns

Comprehensive discussion of findings and treatment options
If a scale and polish is recommended, it will be discussed and scheduled as a separate appointment following the Full Examination.
Complimentary Teeth Whitening Trays
Complimentary take-home teeth whitening trays may be offered in conjunction with the Full Examination, please see below for the terms and conditions in relation to this offer.
Treatment Planning and Recommendations

Acceptance of treatment is entirely optional and not required on the day

Costs, options, and timeframes will be discussed prior to any treatment being undertaken
Appointment Structure

Ongoing 6- or 12-monthly examinations are shorter and tailored based on individual needs

Avenue Dental – Complimentary Teeth Whitening Offer Terms and Conditions
Offer Details
New eligible patients of participating Avenue Dental practices are entitled to receive a complimentary at-home teeth whitening treatment (“Teeth Whitening Kit”), including custom-fitted trays with syringe whitening gel, subject to the Terms and Conditions outlined below.
Offer Period
This offer is valid from 01/01/2017 to 20/12/2026. Patients must fulfill all eligibility requirements and schedule their Comprehensive Examination within this timeframe to qualify for the complimentary Teeth Whitening Kit.

Eligibility Requirements
To qualify for the complimentary Teeth Whitening Kit Offer, patients must meet the following conditions:
- New Patient: Patients must be new to Avenue Dental, indicating they have not previously visited any of the participating Avenue Dental locations for over a 2 year period.
- Comprehensive Examination: Patients must undergo a comprehensive dental examination, including x-rays, or have their first full examination with a participating Avenue Dental practice. Comprehensive Examinations including x-rays are not complimentary.
- Age Requirement: Patients must be over the age of 18 years to be eligible for the Offer.
- Assessment: Following the Comprehensive Examination, patients must be deemed by Avenue Dental to be suitable for teeth whitening without any complicating dental health issues. Patients diagnosed with dental decay during the Comprehensive Examination are not eligible for the complementary Teeth Whitening Kit. In such cases where decay is detected, patients will be advised to address any decay issues before becoming eligible for this Offer. Please see eligibility considerations below.
- Consent: Eligible patients must provide informed consent for the Teeth Whitening Kit and treatment, indicating their understanding of the procedure, its associated risks and benefits, and their agreement to undergo the treatment voluntarily and at their own risk.
Offer Limitations
- This Offer is limited to one complimentary Teeth Whitening Kit per new eligible patient and is non-transferable.
- The Offer does not include any in-chair or Zoom whitening procedures. The Teeth Whitening Kit provided is for at-home use, where patients self-administer the whitening gel using custom-fitted trays. No supervision by a dental professional will be offered by Avenue Dental during the self-administered whitening process.
- The Offer cannot be combined with any other promotions offered by Avenue Dental.
- Avenue Dental reserves the right to modify or terminate this Offer at any time without prior notice.
Treatment Information and Disclaimers:
Please be aware of the following:
- Results Vary: While teeth whitening is generally safe and effective, individual results may vary based on factors such as the starting shade of the teeth, the frequency of use, and adherence to aftercare instructions. Avenue Dental cannot guarantee specific results.
- Potential Complications: Potential complications of the treatment may include gum/lip irritation, tooth sensitivity, allergic reactions and the development of spots or streaks on teeth. It is essential to follow the instructions provided with the Teeth Whitening Kit carefully and consult with a dental professional if any adverse effects occur. Avenue Dental shall not be held liable or responsible for any potential complications that may arise as a result of the at home Teeth Whitening Kit.
- Temporary Nature of Results: Teeth whitening results are not permanent and may regress over time, especially with exposure to staining agents like coffee, tea, and tobacco. To maintain results, it is important to follow aftercare instructions and avoid or minimise consumption of staining substances.
- Eligibility Considerations: Certain individuals, such as pregnant or breastfeeding women, those under 18, individuals with specific dental conditions, or allergies to product ingredients, may not be eligible for the treatment. It’s important to consult with your physician or dentist before using any Teeth Whitening Kit, especially if you are taking photoreactive drugs or have undergone recent dental procedures.
- Aftercare Instructions: To maintain results and minimise potential risks, please follow the aftercare instructions provided with the Teeth Whitening Kit carefully.
- Responsibility for Loss or Damage: While Avenue Dental takes care to provide durable and high-quality trays in the Teeth Whitening Kit, we do not guarantee the durability of the trays. Replacement trays can be remade upon request, but the full cost of replacement, including materials and labour, will be borne by the patient.
- Not a Substitute for Professional Care: The Teeth Whitening Kit is intended for cosmetic purposes only and is not a substitute for professional dental care.
Patient Consent
By accepting the Offer at any participating Avenue Dental practice, patients acknowledge that they have been fully informed about the Teeth Whitening Kit and treatment, including its risks, benefits, and alternatives. They agree to voluntarily self-administer any teeth whitening treatment at home and acknowledge that they have had the opportunity to ask questions and have them answered satisfactorily by Avenue Dental. Patients acknowledge and accept all disclaimers outlined in these Offer terms, thereby releasing Avenue Dental from any and all liability related to any potential complications arising from Teeth Whitening Kit treatment.
Data Protection and Privacy
Avenue Dental is committed to safeguarding the privacy and security of personal information. Please refer to our website terms and conditions and privacy policy for more information.
When using third party offers such as Afterpay, Humm, ZipMoney, DVA and CDBS, we must abide by the individual terms and conditions set out by these offers. It is your responsibility to contact the third party dental offers you want to use regarding their terms and conditions, especially the benefits they provide and limits and restrictions on these. Avenue Dental does not accept responsibility when they do not pay the benefits that you think they should. You are responsible for paying your account if they do not pay us on your behalf.

Avenue Dental has no control over what your private health insurance pays towards your dental treatment. We can process your health fund card on our HICAPS machine as a complimentary service. When we do this, they will pay any benefits to us directly on your behalf to save you having to claim back the benefits yourself after paying us in full. However, what your health fund pays towards your dental treatment is entirely between you and your health fund. It is your responsibility to contact your health fund regarding their terms and conditions, especially the benefits they provide and limits and restrictions on these. Avenue Dental does not accept responsibility when your health fund does not pay the benefits that you think they should. You are responsible for paying your account whether your health fund contributes to it or not.
